What this website collects
The consultation form on this website collects contact information only: your name, phone number, email address, your preferred contact method, the service you are interested in, and an optional message. Please do not include personal medical details in the form. The form is not for medical emergencies.
This policy covers this website only. Information you share as a patient of the practice is protected under the practice's Notice of Privacy Practices, which you receive when you become a patient.
How your information is used
When you submit the form, your information is emailed directly to the practice so we can contact you about scheduling a consultation. It is used for that purpose only. We do not sell your information, share it with advertisers, or add you to marketing lists you did not ask for. Form submissions are not stored in a database on this website.
Text messaging (SMS) terms
If you provide your phone number and consent to receive text messages, Apex Wellness may send you text messages related to your consultation request and your care, such as scheduling, confirmations, and reminders. Message and data rates may apply, and message frequency may vary. You can opt out of text messages at any time by replying STOP or UNSUBSCRIBE to any message from us, by submitting the opt-out form on our website, or by calling (402) 314-8407. Reply HELP to any message for assistance. Opting out of text messages does not affect your ability to receive care.
We do not share your mobile information
No mobile phone numbers, text messaging originator opt-in data, or consent information will be shared with or sold to third parties or affiliates for marketing or promotional purposes. Text messaging opt-in data and consent are used only to communicate with you as described above and are never transferred to any other party for their marketing use.
Analytics and cookies
This site does not use advertising trackers, advertising cookies, or social media pixels. We currently use no visitor analytics.
Service providers
We use a small number of service providers to operate this website: a hosting provider that serves the site and an email delivery provider that transmits form submissions to the practice. Our website also offers online appointment booking provided by a third-party scheduling platform under a business associate agreement with the practice.
